Volunteer Requirements

Below are forms needed to volunteer at school- these forms are to be completed each year. Please read the requirements below for each type of volunteering before opening form.

SPECIAL EVENT(S) | On-line Form only
CLASSROOM/ON-CAMPUS VOLUNTEER | Required: Valid Tuberculosis (TB) Clearance
DRIVER (Field trip or Sports) | Required Field Trip Driver Form, Copy of Driver’s License & Copy of current Auto Insurance Declaration, *** all drivers are required to be screened through a sex offender registry. https://www.meganslaw.ca.gov/
COACH | Required: Valid Tuberculosis Clearance, Fingerprint/Criminal Background Check, Valid CPR & First Aid Certificates, Clearance from District Level Designee
INDIRECTLY SUPERVISED | Required: Fingerprint/Criminal Background Check (If this volunteer position is long-term, a Valid Tuberculosis (TB) Clearance is also required)
